The 8-page document includes:
A description of the clinical examination of neurological function.
This includes major symptoms, questionnaire, mental status assessment, cranial nerve assessment, motor, sensory and cerebellar function assessment, reflex assessment, basic neurological assessment as well a checklist helps memory. in normal format (8.5 x 11) and in small format (plastic sheet approximately 4 x 6 in).
